JOB DUTIES: The Health Information Management Department is responsible for the maintenance and management of quality medical records and preparation of statistical data as it pertains to hospital patients.

This position is responsible for independently performing the para-professional functions in a health information department which include but not limited to assembling and analyzing medical records for completeness and accuracy, assisting customers with requesting release of information, and performing other related duties as required in accordance with established HIM and Medical Staff procedures. This position may be required to work shifts, weekends, and holidays.

MUST BE AVAILABLE TO WORK SHIFTS AS NECESSARY BASED ON HOSPITAL OPERATIONS; SERVICES ARE PROVIDED 24/7.

THE MINIMUM QUALIFICATION REQUIREMENTS ARE: Applicants must meet all of the following requirements. Please note that unless specifically indicated; the required education and experiences may not be gained concurrently. In addition, qualifying work experiences are based on a 40-hour work week.

EDUCATION: High School diploma or equivalent.

EXPERIENCE:

Medical Records Experience: Two (2) years of experience in a health information management department of a hospital or allied medical care facility, or in an allied health or medical program (e.g., nursing unit, admissions dept., clinic, etc.), which included such duties as filing medical records, posting data onto medical records, cross-indexing data, compiling numerical data from medical records or reports, typing/transcribing medical reports, and work of a comparable nature. The experience must have demonstrated knowledge of medical terminology, office practices and procedures pertinent to medical records processing and maintenance, common office appliances and equipment; and the ability to perform arithmetic computations, follow oral and written instructions, operate various kinds of office equipment and deal tactfully and effectively with others.

Specialized Experience: One (1) year of experience in a health information management department of a hospital or allied medical care facility which involved the performance of paraprofessional health information management work requiring knowledge of the nature and uses of medical records, medical terminology and established health information management classification, coding, filing and indexing systems; and the ability to analyze and file medical records and prepare reports and summaries from medical records.

Substitutions Allowed:

  1. Excess Specialized Experience may be substituted for Medical Records Experience.
  2. Successful completion of an associate degree in a health information technology program (HIT) accredited by the Commission on Accreditation for Health Informatics and Information management Education (CAHIIM) or recognized by the American Health Information Management Association (AHIMA), may be substituted for all of the requirements for the class Health Information Technician I.
  3. Successful completion of an associate degree in a health information technology program (HIT) in a foreign country whose professional association has an agreement of reciprocity with the American Health Information Management Association (AHIMA) may be substituted for all the requirements for the class Health Information Technician I.
  4. Successful completion of the correspondence course or the AHIMA Independent Study Programs in health information technology (HIT) and an associate’s degree from an accredited college may be substituted for all of the requirements for the class Health Information Technician I.
  5. Successful completion of a health information technology program of less than two years duration at a post-secondary institution accredited by the American Health Information Management Association may be substituted for the Specialized Experience on a month-for-month basis.
  6. Possession of a current Registered Health Information Technicians (RHIT) certificate received through successful qualification on the national accreditation examination administered by the American Health Information Management Association may be substituted for all of the requirements for the class Health Information Technician II and I.

LICENSURE: None required.

JOB DUTIES: The Information Technology Department provides for integrated, computer-assisted recording, classifying, summarizing, transmitting, storing and retrieving of clinical and management information. Ensures internal and external communication capabilities including phone, fax, and radio.

This position works closely with the IT Manager, IT staff, HHSC Technical Services Director, and users to determine network and PC needs. This position makes recommendations and decisions regarding the design and use of hardware and software for the network, servers, PC’s, network operating systems, network applications, desktop operating systems, and desktop applications. This position will also propose and implement cost-efficient and reliable solutions to address business issues and produce desired results, while adhering to departmental and corporate standards. May require standby call and call back duty.

EXPERIENCE: Three and one half (3 ½) years of progressively responsible work experience in information technology which involved the systems analysis, design, programming, security and access management, database design and management, consulting, and/or installing and maintaining computer software applications, hardware, telecommunications, and web or network infrastructure equipment. The work must have demonstrated knowledge and understanding of information technology principles, methods, techniques and practices; computer server, mini and/or mainframe operating systems. For the IT Specialist IV and higher levels, at least one year of the required specialized experience must have been at the fully competent level comparable to the IT Specialist III level.

JOB DUTIES:   The primary duties of the Account Clerk II position are to submit secondary outpatient bills to third party payers; record and manage posting of payments received through the mail; function as a telephone call staff to resolve patient and third party inquiries and re-direct for callback to other Patient Financial Services Staff; to open and prepare correspondence for scanning and or filing respond to written inquiries for information; manage returned mail; manage reports and hardcopy UB04 distribution; postage metering, scanning of and the reproduction of scanned images, mailing of refund checks , documenting activities within the related patient account notes and other duties as assigned.  MUST BE AVAILABLE TO WORK SHIFTS AS NECESSARY BASED ON HOSPITAL OPERATIONS; SERVICES ARE PROVIDED 24/7.

In addition, this position will determine staffing methodology, direct and coordinate the staff activities, monitor staff performance, assure competencies and provide for the educational needs of all staff members. JOB DUTIES:  The Clerk IV is responsible for independently providing a variety of clerical and unit assistance to support the professional staff.  This position will perform independently and uses good judgment and common sense when prioritizing tasks assigned during emergency situations.  The Clerk IV performs functions in accordance with the established policies and procedures of the hospital and is knowledgeable of regulations governing Medicare, Medicaid and Collective Bargaining Agreements.  The subject position is subject to shift work, works weekends and holidays.  MUST BE AVAILABLE TO WORK SHIFTS AS NECESSARY BASED ON HOSPITAL OPERATIONS; SERVICES ARE PROVIDED 24/7.

THE MINIMUM QUALIFICATION REQUIREMENTS ARE: Applicants must meet all of the following requirements. Please note that unless specifically indicated; the required education and experiences may not be gained concurrently. In addition, qualifying work experiences are based on a 40-hour work week.

Education: Graduation from high school preferred.

General Experience:  Two (2) years of clerical work experience which involved the performance of a variety of clerical tasks and demonstrated knowledge of English grammar, spelling, arithmetic, common office appliances and equipment (e.g., copying machines, word processors, etc.); and the ability to read and understand oral and written instructions, carry out procedures in clerical work systems, communicate effectively orally and in writing; deal tactfully with others; operate common office equipment; and organize and prioritize work tasks.

Non-Qualifying Experience:

Experience limited to the performance of simple and repetitious clerical work which primarily involved a short cycle of tasks and manual dexterity shall not be considered qualifying. Examples of such non-qualifying work include but is not limited to such tasks as wrapping and unwrapping articles; opening envelopes, folders and similar containers, affixing tags and labels; arranging, rearranging or tidying up office desks and equipment; delivering, distributing or collecting correspondence or parcels.

Substitution of Education for General Clerical Experience:

1. Successful completion of a substantially full-time equivalent clerical curriculum of a duration of a year or longer, leading to a degree or diploma at an accredited community college, or at a business or technical school which included courses in basic English, arithmetic, general clerical procedures, and office machines will be deemed to have met the experience requirements for one (1) year of experience.

2. Successful completion of a substantially full-time equivalent clerical curriculum leading to a degree, diploma, or a certificate of achievement at an accredited community college, business school or technical school which was for a period of less than one (1) year will be substituted for experience on a month-for-month basis provided the training included courses in basic English, arithmetic, general procedures, and office machines.

3. Partial Completion of Clerical Training:

a. Completion of half a school year of substantially full-time clerical curriculum at an accredited community college, or business and/or technical school which included courses in basic English, arithmetic, general clerical procedures, and office machines may be substituted for six (6) months of experience.

b. Completion of one (1) school year (a program of more than one year in length) of substantially full-time clerical curriculum at an accredited community college, or business and/or technical school which included courses in basic English, arithmetic, general clerical procedures, and office machines will be deemed to have met the requirements for one (1) year of experience.

4. Education in an accredited university in a baccalaureate program with courses in English composition and college mathematics may be substituted for general clerical work experience on the basis of 15 semester hours for six (6) months of experience, up to a maximum of two (2) years.

*Applicants for Clerk IV and V must demonstrate possession of Supervisory Aptitude. Supervisory aptitude is the demonstration of aptitude or potential for the performance of supervisory duties through successful completion of regular or special assignments which involve some supervisory responsibilities or aspects; by serving as a group or team leader, or in similar work in which opportunities for demonstrating supervisory capabilities exist; by completion of training courses in supervision accompanied by application of supervisory skills in work assignments; or by favorable appraisals by a supervisor indicating the possession of supervisory potential.

Kohala Hospital is a Critical Access Hospital (CAH) located in the rural town of Kapaau (North Kohala).  We opened our doors to patients on April 1, 1917.  At that time, it was a 14-bed facility.  The cost of hospitalization at that time was $1.50 per day.  In 1962, Kohala Hospital was relocated into a structure consisting of 26 inpatient beds providing both long-term and short-term acute care.  Today, Kohala Hospital employs 55 employees and provides 24-hr emergency care, laboratory services, radiology services, and Skilled Nursing & Intermediate Care to our community.
