SALARY AMOUNT:  75% OF $8,061 PER MONTH (SR-20; BU:09)

 

JOB DUTES:

Patient Navigator

  1. Organizes and facilitates pre-operative patient classes, either in-person or virtually, with the objective of preparing patients for outpatient surgical procedures and recovery expectations.
  2. Completes at-home safety checks, ensures support coach preparedness, reviews and collects patient documentation, provides appropriate patient education materials.
  3. Completes post-operative follow-up calls with patients, providing guidance and support through rehabilitation.
  4. Collaborates closely with physicians, surgical team, physical and occupational therapists to ensure patient is supported throughout surgery, recovery and rehabilitation periods.
  5. Provides a point of contact for patients as they navigate from preoperative preparation to the day of surgery and through their rehabilitation period.

The RPN III coordinates nursing care for patients, participates in patient and family teaching and provides leadership by working cooperatively with other staff and departments in maintaining standards of nursing care; follows established policies, procedures or specific instructions.  The position may work on rotating shifts, extended workdays with work on weekends and holidays; requires standby call and call back duty.  While on standby or call back duty, the incumbent of the position is required to report to work within 30 minutes of receiving call.

EDUCATION, EXPERIENCE, CERTIFICATION, LICENSURE

Education: Graduated from an accredited school of nursing.  While not required, graduation from an accredited nursing program with a Bachelor’s Degree from an accredited college or university is highly desirable.

Experience: One-half (1/2) year of progressively responsible professional nursing work experience. Of the one half (1/2) year of experience, one-half ( ½ )year in a critical care setting where the employee independently provided nursing care to pre-operative and post-operative patients is highly desired. Such experience must have been gained within the last two (2) years is preferred.

Certification: Current Basic Life Support (BLS) certification at time of appointment, Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certification are required within six (6) months of employment.

Licensure: Possession of a current license to practice as a professional nurse in the State of Hawaii.

 Required Forms & Documentation: Applicants must submit evidence of the appropriate training (e.g. official transcript or diploma and certifications) to be given credit for education.  A legible photocopy will be accepted; however, Hawaii Health Systems Corporation, Kona Community Hospital reserves the right to request an official copy.

 STEPS TO AN ADMINISTRATIVE REVIEW, SUBSEQUENT APPEALS: If you do not agree with a decision made by the Employment Office as to your non-qualification or non-selection for a position, you may complete a Request for Administrative Review form (available on the HHSC website) or you may submit a written request within twenty (20) days from the date of your sent notice to the Regional Chief Executive Officer/Designee. Your letter requesting the Administrative Review must include 1. The job title(s) and recruitment number(s), 2. the specific reason(s) you are requesting the review noting if there is a statute or rule violation, and 3. any additional information you want to submit to substantiate your request. If you do not submit your request within the twenty (20) days deadline, no Administrative Review will be conducted. Since the Administrative Review is a prerequisite to subsequent steps, failure to utilize this process will make you ineligible for subsequent appeals. The administrative review, formal complaint and/or appeals hearing will not necessarily postpone the recruitment process and/or rescind a selection.
